NHS Health Checks

The NHS Health Check is a free health assessment designed to identify your risk of developing conditions such as heart disease, stroke, type 2 diabetes, kidney disease and dementia. Early identification allows us to help you reduce your future health risks.

What does an NHS Health Check include?

  • Blood pressure measurement
  • Height and weight
  • Body Mass Index (BMI)
  • Cholesterol testing
  • Diabetes risk assessment
  • Lifestyle assessment including smoking, alcohol, diet and exercise
  • Personalised advice to improve your health

…and any additional assessments that may be appropriate for your individual circumstances.

Who is eligible?

Most adults aged 40 to 74 years who do not already have certain long-term conditions are eligible for a free NHS Health Check every five years.

How to access this service

If you believe you are eligible, please submit an Online Book Medical Request. All requests are reviewed by one of our GP triage doctors, who will arrange the appropriate appointment or advise if you are not currently eligible. Eligible patients may also receive an invitation when their Health Check is due.
